Job Description:
Conduct thorough psychosocial assessments for new clients to gather detailed personal, social, emotional, and behavioral history.
Collect and document information on clients' needs, concerns, and challenges.
Identify immediate needs and risks, including safety concerns (e.g., suicidal ideation, abuse, neglect).
Provide initial support, including resource referrals, counseling, and crisis intervention.
Develop comprehensive case summaries and treatment recommendations based on assessments.
Participate in team meetings, case consultations, and supervision to improve service delivery.
Collaborate with multidisciplinary teams to develop and implement appropriate care plans.
Maintain accurate, up-to-date, and confidential client records in compliance with ethical standards and regulations.
Exercises discretion and confidentiality while handling sensitive situations.
Reviews, analyzes, interprets, and/or prepares federal and state laws, rules, regulations, Code of Ethics, policies and procedures as related to clinical social work.
Interprets, monitors, and record client progress according to measurable goals described in treatment and care plan.
Has moderate level of authority to refer patient, client, or family to community resources to assist in recovery from mental or physical illness and to provide access to services such as financial assistance, legal aid, housing, job placement or education.
Performs other related work as needed.
Preferred Qualifications
Experience:
Working for a large complex healthcare organization.
Experience in a fast-paced outpatient, community, or clinical setting.
Knowledge of diverse populations and cultural competence in delivering therapy.
Experience with crisis intervention and trauma-informed care.
Licenses and Certifications:
LSW or LCSW Illinois licensure or license eligible.
LCSW preferred.
Technical Skills or Knowledge:
Knowledge of federal and state laws, rules, regulations, code of ethics, policies, and procedures as related to social work.
Preferred Competencies
Strong interpersonal skills, including active listening, empathy, and the ability to build rapport with clients from diverse backgrounds.
Knowledge of mental health disorders, psychosocial assessments, and evidence-based interventions.
Proficient in using assessment tools and documenting findings.
Ability to work independently and as part of a multidisciplinary team.
Knowledge of community resources and ability to make appropriate referrals.
Strong communication, organizational, and time-management skills.
Ability to handle sensitive and confidential information with professionalism and integrity.
